Scheduling an Appointment

To schedule an appointment:

Complete our forms from Patient Forms page
If you are under 18 years old, complete all forms under Patient Forms except for Pre-Assessment Adults.
If you are over 18 years old, complete all forms under Patient Forms
If you have been seen by another psychiatrist, have your previous provider forward your records to our office.
After the above steps have been completed, please call us at (712) 234-0220 to schedule your appointment.
At the time of your appointment bring your insurance card and driver license.

We Accept All Major Insurance

Medicare, Blue Cross Blue Shield, Aetna, UBH, CHIP, and Iowa, South Dakota, and Nebraska Medicaid, as well as most other insurances accepted in the Siouxland area.
